We make your DOT examination experience quick and easy! Our offices offer same day, walk-in service during the appointed times (always a good idea to call ahead to limit your waiting time). We value your time so intake paperwork can be pre-filled through our portal when your apt. is confirmed. Everything can be done electronically!

DOT Exams

Now offering Driver Friendly D.O.T. Medical Cards for CDL and Hoisting Licenses
As part of our dedication to comprehensive patient care, we offer DOT exams for commercial driver’s licenses and physicals for drivers who don’t require a CDL but who haul enough weight to require a DOT examination.

A basic DOT exam does not include drug and alcohol testing. Drug and alcohol testing requires a separate test and is not included in the cost of a basic DOT exam. We are currently not able to offer this testing but will accept copies of verifiable testing from outside laboratories if this is a requirement for you.

We have ample parking for commercial vehicles including tractor-trailer rigs as there is a full-sized parking lot that wraps around our building (we are in the back building behind the EZ-Pass service center).

What to expect

Driver health history
Before meeting with the examiner, you must fill out the top part of the DOT physical form which is available as a fillable PDF here. Instructions for the form can be found here. The first section has room for your name, social security number, birth date, driver’s license number, address, and telephone number. The second section asks about your family medical history, past illnesses, and overall health. Be sure to answer each question truthfully.

DOT / CDL Physical Examination
Our provider will review your information and perform a basic urinalysis test as well as perform a series of assessments to check your blood pressure, vision, hearing, digestive, and mental health. The exam typically takes about 30 minutes.

Paperwork
Upon passing, you will receive a medical certificate that is valid for between three (3) months and two (2) years, depending on your overall health. We will fill out all the necessary paperwork to process the exam results and give you a laminated medical card to carry with you as well as any copies of the certificate you may need. If your employer needs a copy sent directly from us, you can sign a waiver and have them request that directly or we can send it to an email/fax number that you provide. We’ll also keep a scanned copy of the exam in your file at our office.
Should you not qualify for your medical certificate for any reason, our provider will explain what needs to be addressed to meet current FMCSA guidelines.

About Us

Cindy Porter grew up on the West coast, moving from Washington to Alaska and back to Oregon before beginning her trek across the US to finally end up here in Massachusetts. She received her Bachelor of Science from Worcester State University and then went on to complete her APRN Program at MCPHS in Worcester. She still works full-time as a hospitalist but has also started her own business helping people achieve optimal health through alternative therapies. As a former school bus driver, she is aware of the struggle to maintain a current CDL. When away from the office, Cindy enjoys spending time with her family, visiting the grandchildren in Florida as well as scuba diving, and enjoying the scenery from her motorcycle.

You must meet the federally-mandated medical standards to get a Commercial Driver’s License (CDL). You are physically qualified to drive a commercial motor vehicle if you either:

To ensure that you meet these medical standards, you must pass a Department of Transportation (DOT) medical examination. Only a licensed medical examiner may conduct your examination.

Certified medical examiners can be found on the Federal Motor Carrier Safety Administration (FMCSA) National Registry. Medical examiner’s certificates are valid for a maximum of 2 years.

To operate a commercial motor vehicle with a gross vehicle rating exceeding 10,000 pounds in interstate commerce, a medical examiner’s certificate may also be required, even if you do not have a CDL.

Tips for managing blood pressure
Anticipation of a physical can be anxiety-provoking and we understand how important it is to pass your DOT exam. To help prepare yourself for the appointment, here are things you can do in the days and weeks leading up to your exam:

  • Take your medication as prescribed and on schedule every day
  • If you forget a dose, take it as soon as possible (if applicable) and let us know if you’ve recently missed a dose
  • Reduce your salt, caffeine, nicotine, and sugar intake

Urinalysis
Right before your exam: Make sure you drink a glass of water and come in with a full bladder, as you will have to provide a urine sample.

  1. Photo identification and medical records
  2. Eyeglasses, contact lenses, or hearing aids if you use them
  3. A list of your current medications, including strength and dosages, as well as the name and contact number of the doctor who prescribed them.
  4. Complete Driver Information and Health History sections of the Medical Examination Report Form. If you are unable to print the form, you can email it to us for printing and sign in-office during your visit.
  5. If you take medications for such things as pain, psychiatric conditions, attention deficit disorder, or smoking cessation, bring a letter from your treating doctor indicating the medication and condition being treated and affirmation that it will not interfere with the operation of a commercial motor vehicle.
  6. Medical records for conditions such as diabetes, cardiac disease, sleep disorders, neurological conditions, etc. this means a copy of your most recent stress test (for heart conditions) and a copy of your latest sleep test result (for sleep apnea).

CDL drivers must take the DOT physical every two years. Should you have a condition that needs closer monitoring, such as high blood pressure, you may be required to get a physical more often.

A person is physically qualified to drive a commercial motor vehicle if that person does not have any of the following conditions which may interfere with the ability to control, operate, and drive a commercial motor vehicle safely:

  • No loss of foot, leg, hand, or arm.
  • No impairment of hand or finger which interferes with power grasping, or no impairment of arm, foot, leg which interferes with ability.
  • No history or clinical diagnosis of diabetes that currently requires insulin for control.
  • No current diagnosis of myocardial infarction, angina pectoris, coronary insufficiency, thrombosis, or other cardiovascular disease known to be accompanied by syncope, dyspnea, collapse, or congestive cardiac failure.
  • No history or clinical diagnosis of respiratory dysfunction likely to interfere with ability.
  • No current diagnosis of high blood pressure likely to interfere with ability.
  • No history or clinical diagnosis of rheumatic, arthritic, orthopedic, muscular, neuromuscular, or vascular disease which interferes with ability.
  • No history or clinical diagnosis of epilepsy or any other condition which is likely to cause loss of consciousness or any loss of ability.
  • No mental, nervous, organic, or functional disease or psychiatric disorder likely to interfere with ability.
  • Has distant visual acuity of at least 20/40 in each eye, or visual acuity separately corrected to 20/40 or better, field of vision of at least 70 degrees horizontally, and the ability to perceive red, green, and amber.
  • Ability to perceive a forced whisper in the better ear at no less than five feet, with or without the use of a hearing aid.
  • Does not use a Schedule I drug, amphetamine, narcotic, or any other habit-forming drug.
  • Has no current clinical diagnosis of alcoholism.
